Founded in 2020, New Breed Creation Life Center (New Breed) is a 501(c)(3) faith-based nonprofit organization located in Jacksonville, Florida. We are dedicated to addressing the critical behavioral health, substance use, and housing needs of underserved and high-risk communities across Northeast Florida.

Our mission is rooted in health equity, recovery, and empowerment. We believe every person deserves access to comprehensive care that supports healing and personal transformation—regardless of their background or circumstances.

Since our founding, New Breed has grown into a trusted, community-centered provider of integrated behavioral health services. Our approach combines evidence-based treatment with spiritual guidance, case management, and strong community partnerships to promote long-term wellness and stability.

Our Services

New Breed offers a full continuum of care through the following core services:

Behavioral health counseling and recovery services
Substance use prevention, intervention, and treatment
HIV/STI testing, counseling, and linkage to care
Intensive case management and wraparound support
Youth and family wellness programs
Housing support and coordination for homeless populations

Each program is tailored to meet individuals where they are—offering compassionate care, cultural humility, and trauma-informed support.

Adult Mental Health & Substance Use

Co-Occurring Disorders Outpatient Program: Provides individual and group therapy to adults with co-occurring mental health and substance use disorders in the office and community settings. We utilize an integrated approach to treatment where both disorders are addressed simultaneously.

Teleservices allows patients to see a therapist from the comfort of their own home, while sitting in the car, or on a lunch break. It’s Face-Time healthcare in the palm of a hand, from anywhere! All one needs to participate in teleservices is an electronic device with a camera, audio, and internet. Here are just a few of the many benefits: Allows patients to see a therapist without hassle or a long commute. Provides invaluable conveniences to busy patients, like missed work, less commute, no childcare arrangements. Delivers more costeffective healthcare

Children Mental Health & Substance Use

Psychiatric and Medication Management: Psychiatric evaluation and medication monitoring for children and adolescents for whom psychotropic medication may be beneficial toward achieving stability and optimal functioning. Nursing services monitor vital signs and provide education to each family regarding prescribed medications.

Psychiatric

This outpatient service provides psychiatric assessment and medication treatment for psychiatric disorders, both for children and adults.

Targeted Case Management

Youth Case Management: Utilizes the Wraparound process as a way to ensure that children with behavioral health issues and their families have access to a comprehensive array of services that address physical, emotional, social and educational needs. This process is based on individualized, strength based, needs-driven planning and service delivery. Case Managers help families access support/services and identify relevant support systems through assessment, planning, linkage, advocacy and monitoring activities.
