What Is Intensive Case Management?
Intensive Case Management (ICM) is a client-centered service that provides one-on-one support to individuals who need help navigating multiple systems — including healthcare, housing, employment, education, and legal aid.
Your dedicated case manager will:

Work with you to create a personalized care plan

Help you access behavioral health and medical services

Connect you to housing, food, and financial assistance

Support you with employment and education resources

Advocate on your behalf with service providers

Help you build skills for independence and long-term success
Wraparound Support Services
Our wraparound model means you're not just getting one service — you're getting a team approach focused on your whole well-being. We coordinate care across all areas of your life, including:

Mental health and addiction services

Basic needs (food, clothing, hygiene, shelter)

Life skills and goal setting

Crisis intervention and de-escalation support

Transportation coordination

Legal and benefits navigation (Medicaid, SSI, etc.)
